• Suffering from Extreme Financial Hardship
• Assistance with Household Challenges (Food, Clothes, Furniture, etc.)
• Emergency Services (Assistance with utility bills, etc.)
• Families that have been impacted by COVID-19
  Must Be Able To Provide Proof of Need

Who can apply?

• Households looking to apply must meet at least one the following requirements:
• Families with Children o Seniors
• Foster Youth/Relative Caregivers

How to apply?

• Name, Address, Phone Number, Email Address, Number of Children
• List of Items Needed
• Summary on the Hardships That Led To Needed Assistance
• How The Assistance Will Impact Your Family
